Light Adjustable Lens and Near Vision: What to Actually Expect

Quick answer: The Light Adjustable Lens gives excellent distance vision, and most patients get functional near vision through blended vision, where one eye is fine-tuned to favor near. In the published registry of the current lens, where 89% of patients chose blended vision, 82% read 20/25 or better at 40 centimeters with both eyes open. Most of my patients use their phone comfortably without readers.

How an adjustable lens gets you near vision

The current version of the lens I implant, the LAL+, works like an extended depth of focus lens: strong distance, good intermediate. The near vision strategy is blended vision. One eye, usually dominant, is targeted for distance. The other is adjusted to be slightly nearsighted, which sharpens things up close. With both eyes open, your brain merges the two and you get a range.

What makes the LAL different from doing this with any other lens is that you don't have to commit in advance. We can set up blended vision, let you live with it for a week, and adjust if it isn't right. If you hate the near offset, we can walk it back toward distance. No other lens can be adjusted after it's in your eye.

The numbers, honestly

In the registry of the current-generation lens, with both eyes open and no glasses: 89% of patients saw 20/20 or better at distance, and 82% saw 20/25 or better at near measured at 40 centimeters. Nearly all of those patients were set up with blended vision, so these are both-eyes-working-together results, which is how you actually live.

Will my phone and menus be comfortable without glasses?

From my own patients: phone and menus are generally comfortable without glasses. Very small print, dim restaurants, and long reading sessions are where some people still reach for readers. If your goal is reading tiny print for hours glasses-free, a trifocal does that better, at the cost of more halos at night. That trade is the real decision, and I'd rather you make it knowingly.
