Cataract Surgery After LASIK: What Changes, What Doesn't, and How We Fix It

Quick answer: LASIK doesn't change the cataract operation itself, but it makes choosing your lens implant power less accurate: roughly two out of three eyes land on target with standard formulas, versus about 80 percent in eyes that never had surgery. In an eye that has never had surgery, modern formulas land within half a diopter of the target about 80 percent of the time. If you've had LASIK, that drops to roughly two out of three. Here's why, and what we can do to get those odds back.

I have a lot of patients who come in and ask: "Is my cataract surgery going to be more difficult because I had LASIK?" I tell them the answer to that is no, and yes.

Let's start with the basics. LASIK reshapes the cornea, the clear covering on the front of the eye, so you can get out of your glasses. A cataract is when the lens inside your eye gets cloudy, and the only way we fix it is by removing that lens and replacing it with a lens implant.

The "no": your surgery is the same

The actual surgery, removing the lens and putting in the implant, is exactly the same whether you've had LASIK or not, because LASIK doesn't affect the lens inside your eye. I tell my patients: in terms of the surgery itself, LASIK has no effect. Your surgery should go the same way it would for someone who never had LASIK.

The "yes": the lens math gets harder

The way we choose a lens in cataract surgery is by taking a bunch of measurements of the shape and size of your eye and plugging them into formulas. The formula's job is to predict which lens power fits your eye. In an eye that has never had surgery, these formulas are very accurate: with modern formulas, we land within half a diopter of the target about 80 percent of the time. Half a diopter is close enough that most people are happy with their distance vision.

If you've had LASIK, that number drops substantially. Even with the brightest minds working on these formulas, LASIK altered the natural shape of your eye, and the formulas have not fully figured out how to account for that. In published studies, post-LASIK eyes land within that half-diopter window somewhere between 55 and 76 percent of the time, depending on the method: call it roughly two out of three.

I want to be clear about what being "off target" means, because patients hear these numbers and imagine something went wrong. It doesn't mean your surgery failed, and it doesn't mean anything is wrong with your eye. It usually just means you need glasses for more things than you were hoping to. ORA: a second opinion during surgery

ORA takes extra measurements of your eye during surgery, after the cataract is already out, and uses those real-time measurements to double-check the math before the lens implant goes in. In the largest published series of eyes with prior LASIK or PRK, 246 eyes, ORA got 67 percent of eyes within half a diopter of target, compared to roughly half using preoperative formulas alone. The Light Adjustable Lens: fine-tuned after your eye heals

The Light Adjustable Lens is a lens we can adjust after it's inside your eye. We choose it using the same formulas as any other lens. Then, about a month after your eye heals, we check whether you have any leftover glasses prescription. If you do, we correct it by shining UV light on the lens, which reshapes it right inside your eye. Nothing touches your eye, and it happens in the office.

That adjustability is what takes care of the error LASIK bakes into the calculations, because the lens gets customized to your eye after it has healed. In a small 34-eye study of the current-generation Light Adjustable Lens in eyes with prior LASIK or PRK, 97 percent ended up within half a diopter of target. That's a better hit rate than formulas achieve in eyes that never had surgery at all. What I tell my patients

The mechanics of your surgery: unchanged. The lens selection: genuinely harder, especially with traditional lenses and formulas alone. If you've had LASIK and you're facing cataract surgery, don't panic. Just make sure you and your surgeon have a plan for the lens calculation, and ask whether ORA or the Light Adjustable Lens makes sense for your particular eye. That's a conversation to have at your consultation, because the right answer depends on your measurements, your goals, and your budget.

Common questions

Does LASIK make cataract surgery riskier?

The operation itself is the same, because LASIK doesn't affect the lens inside your eye. Where LASIK matters is accuracy: choosing the right lens implant power is harder in a post-LASIK eye, which is a precision issue rather than a safety issue.

Why are lens calculations less accurate after LASIK?

The formulas rely on measurements of your eye, including the shape of your cornea. LASIK changed that shape, and formulas haven't fully figured out how to account for it. Accuracy drops from about 80 percent to roughly two out of three with standard formulas.

What are my options if I've had LASIK?

Two main ones. ORA re-measures your eye during surgery to refine the lens choice in real time. The Light Adjustable Lens lets your surgeon fine-tune the lens power after your eye heals; in one large study of post-LASIK eyes, it got 97 percent of eyes within half a diopter of target.

Do I need my old LASIK records?

Bring them if you have them, because they can help the calculations. If they're long gone, don't worry: current methods can work without them.
